5. Primary license (or) NOC
6. Other license
7. Passport (original & color copy -2Nos)
8. All the testimonials ( current & previous) must be with following details
- Period of Work ( from/to - date/month/year)
- Bed Capacity Of Hospital
- Area of Practice/Specialty/Department
- Work performance
- Signed by Nursing Superintendent/ Nursing Director/Hr/Doctor
- It should be on official letter head with seal of the hospital
9. Affidavit (if name in Certificate/Testimonial differ from Passport)
10. Service Gap Letter (if required)
11. Marriage certificate (if married)
